Are you worried about going home from hospital? If you are an in-patient in the RUH Wards or A&E, St. Martin's Hospital, The Mineral Water Hospital or Paulton Hospital and you live in the Bath and North East Somerset area, are 18 years of age or over, and are worried about how you will manage when you get home we could help.

We try to make the return from hospital a little easier for patients, and are able to transport a patient home in our own vehicles from any of the 4 hospitals in BANES and settle patients in.
If needed, we can provide free practical support for up to 4 weeks following discharge, including the following services:
Transport home and settling in
Making sure you are safe, warm and comfortable
Helping with shopping
Helping with light meals or arrangements for a meal
Fetching prescriptions
Transport to GP or hospital appointments
Light cleaning/making the bed
Providing information about other useful organisations 
Popping in for a welfare check
We provide reassurance and support to a patient whilst they are recovering from an illness or operation, and can work alongside professional agencies, families and carers.
We also offer a support service for people in hospital with dementia. Get in touch with our Home from Hospital team (details below) and we can arrange to visit you in hospital.
