Falls Prevention Co-ordinator 

Hours: 13 hours per week

Salary: £6,151 pa based on 13 hours per week

Age UK Cheshire's Falls Prevention Service works to reduce the number of older people who experience falls, particularly aiming to reduce A&E and hospital admissions and the resulting loss of physical health and wellbeing.  By carrying out home visits to individuals and talks to groups, we are able to highlight possible hazardous areas within the home and undertake minor alterations where necessary. 

 

The service is available in Western Cheshire and will operate within the new Falls Pathway.

 

This is a well-established service with funding from Western Cheshire CCG confirmed until March 2015.

We are seeking to recruit a Falls Prevention Co-ordinator to work in our Ellesmere Port office to work alongside a job share partner as a team.  Work will involve home visits, assessing risks and giving promotional talks, as well as the administrative responsibilities for the role.

 

This post is based on working 13 hours per week, to be worked in agreement with the Senior Health & Wellbeing Officer (must include a Monday). Salary is £6,151 per annum for hours worked.

Senior Get Active Co-ordinator 

Hours: 21.75 hours per week

Salary: £12,520 pa based on 21.75 hours per week

Age UK Cheshire run successful health and wellbeing services for older people aged 55 and over, which include Get Active, Falls Prevention, Get Going Together and Peer Health Mentoring. We are seeking to recruit a Senior Get Active Co-ordinator to manage the provision of the Get Active contract in Cheshire West and Chester and a small team of staff and volunteers (currently one staff member).

 

Age UK Cheshire delivers exercise and fitness based programmes to meet the needs and interests of older people, many of whom have long term health conditions. The activities offered are diverse and include: falls prevention classes, cardiac rehab sessions, Tai Chi, Yoga, bowls, walking football, walks, Nordic walking, Golf, horse riding, Zumba Gold, etc.

 

Our aim is to increase physical activity, confidence and self-esteem amongst participants, as well as reducing falls and visits to GPs and hospitals. We also want to empower communities in disadvantaged areas to take responsibility for their own health and wellbeing and support older people to actively engage. 

 

This post is based on working 21.75 hours a week, days to be agreed with the suitable candidate.  This job requires a flexible approach to working hours and some evenings/weekends may be necessary (ideally including a Monday). Salary £12,520 pa for hours worked
