Registered with the Care Quality Commission our Last Years of LIfe Service provides both emotional and practical support to adults with palliative care needs along with supporting their carers and families through a supportive holistic approach.

 

Age UK East London Last Year of Life Service is registered with the Care Quality Commission and funded by Tower Hamlets Clinical Commissioning group.

Age UK East London, and its partner organisation St Joseph’s Hospice Hackney, is able to provide both emotional and practical support to adults 18+ with palliative care needs through a supportive holistic approach. 
 
The support available is:


• Personal Care e.g. hair washing, bathing
• Befriending
• Escorting to GP and Hospital Appointments
• Advocacy
• Domestic Help e.g. laundry, housework
• Shopping
• Information & Advice e.g. completing health related benefit application forms
• Practical Support e.g. garden tidy, moving beds etc.
• Home energy checks e.g. draft proofing, energy advice etc.


In addition, our partner organisation St Joseph’s Hospice Hackney is offering Namaste (therapeutic sensory massage) for patients with advanced dementia.
