We are currently recruiting for: 

The Living Well Project

If you are interested in making a positive difference to the lives of Carers, older people, people living with a learning disability or those living with a mental health condition, we need to hear from you.
An innovative Living Well project is about to commence that will:
Promote people’s health and wellbeing, supporting them to increase their ability to live well and retain their independence
Build new partnerships to support the delivery of person-centred, holistic care planning and co-ordination
Work with Adult Social Care to support those who feel on the edge of crisis, particularly those not eligible for statutory funding
Support people leaving hospital by providing new models of care in the community
The Living Well team needs the following:
Living Well Project Manager - 30 hours per week; £30,000pa pro rata
Living Well Mental Health Advisor - 35 hours per week; £22,000 pa
Living Well Learning Disability Support Officer – 35 hours a week; £20,500pa
5 Living Well Support Officers - 35 hours per week @ £18,000 pa 
Living Well Carers Team Leader – 37.5 hours per week; £24,500pa
3 Living Well Support Workers (Carers) - 35 hours per week; £20,500pa
2 Brokerage Support Service Officer - 35 hours per week; £20,500pa
All roles are initially for 20 months starting Sept/Oct 2017
