Hospital to Home Support Worker - Community

Salary NJC Scale 4: £17,714 pro rata: 49 hours per fortnight

 

As a Hospital to Home (H2H) Support Worker you will be responisble for providing practical and emotional support to individual clients upon discharge from hospital or following attendance in A&E and JAMA.

 

The successful candidate will have experience of assessing individual care needs and providing support, including outreach support, to older people, their families and carers.

 

A full driving license and use of a vehicle for work will be a distinct advntage.

 

You will be based in Leeds.

Hospital to Home Care Coordinator - (H2HCC1.15)

Salary NJC Scale 6: £22,937 pro rata:

There are 2 vacancies for the Care Coordinator. Each vacancy is 56 hours over a fortnight

 

As a Hospital to Home (H2H) Care Coordinator you will be responsible for assessing individual clients care needs and coordinating the support needed, both from our own team and other appropriate service providers.

 

The successful candidates will have the ability to allocate work using a 7 day rota system, develop and maintain good working relationships and ensure the service is provided consistently and to a high standard.

 

The ability to assess the needs of older people and the knowldge of what support is available are essential for this post. Experience of working within a hospital environment would be of a distinct advantage.

 

You will be based at St James Hospital in Leeds.

Cafe Assistant - (CFAS1.15)

Salary £6.50 p/hr 3-4 shifts per week [over 6 days] occasional events and Sundays.

 

As a Café Assistant you will assist in the day to day running of the café and catering services.

 

You will maintain high standards of customer service and food presentation.

 

The successful candidate will have the ability to prapare food and hot beverages throughout the day and provide a high quality of customer service at all times.

 

Experience of working in a busy catering environment is essential for this post.

 

Experience of cash handling and a level 2 food hygiene certificate would be of a distinct advantage.

 

You will be based in the Arch Café, Bradbury Building.
