Age UK Leeds are looking for a Wellbeing Coordinator (Frailty) As a Wellbeing Coordinator  you will be responsible for supporting individuals to develop independence, resilience and enable self management of their health and wellbeing.

The successful candidate will have the ability to complete comprehensive support plans tailored to individual need as well as motivating people in order to sustain changes to their lives which is essential for this post.  

Experience of working with older people who are socially isolated and/or frail would be of a distinct advantage.

Being able to build and foster partnership working with a wide range of services in order to address social and health needs of the individual is essential for this post.

Experience of working with older people who have experienced social isolation would be of a distinct advantage.

Caseworker (Mental Health Hospital Aftercare)

Age UK Leeds are looking for a Caseworker you will be responsible for supporting individuals experiencing mental health difficulties or emotional distress to enable self-management of their mental health and wellbeing.

The successful candidate will have the ability to create support plans tailored to client need and form trusting relationships as well as developing partnership working with a wide range of services. 

Understanding of the issues and barriers that older people face is essential for this post.

Experience of working with older people experiencing mental health issues would be of a distinct advantage.

You will be based at the Bradbury Building, Leeds City Centre
