Help at Home Business Development Officer

Salary: SO1: £25,951: 30 - 37 hours (hours are negotiable)

Do you have experience of setting up and / or managing Help at Home / domiciliary services?

We are looking for an enthusiastic Business Development officer to help us test the assumptions in our business case and launch our new Help at Home Service.

Age UK Leeds is planning to launch a new Help at Home service to undertake household and other supportive tasks for older people to assist them to remain independent and live in their own homes.

The service will be paid for by clients and will operate as a social enterprise. The commercial viability of the service is a priority, both in providing a sustainable service to clients but also in helping to secure a sustainable income for the future of Age UK Leeds.

Digital Angels Community Development Worker

Age UK Leeds provides a range of services and products in the community, which have been specifically developed to meet the needs of older people. We are now looking for a Digital Angels Community Development Worker.

Salary NJC Scale 6 £22,927 pro rata , 21 Hours per week.

As a Digital Angels Community Development Worker you will be responsible for Identifying socially isolated older people & developing effective referral routes into the service. The Provision of one to one IT support to clients in their own homes , identifying their interests & using appropriate methods ( on and off line ) to engage them .Setting up group activities, using volunteers where appropriate. Recruiting, managing & developing the Digital Angels volunteer team and providing regular support & supervision. Building positive working relationships with external partners & key stakeholders in order to develop a high profile for the service within communities to promote Digital Angels. Maintaining accurate records & collect routine data for the monitoring & evaluation of the service.

The successful candidate will have the ability to Influence, persuade and motivate volunteers & clients. Manage multiple priorities against tight deadlines. Work proactively on own initiative & flexibly as part of a close team. Drive & have access to a vehicle for business use. Communicate with a wide range of people. Understand the issues associated with older people have an in depth understanding of computer hardware & software. To understand the roles of volunteers and to understand the principles of Safeguarding.

Experience of Supporting & managing volunteers, Administrative ability e.g. organising meetings , managing diaries and e mails, managing filing systems & databases. Working with a range of IT applications & solutions e.g. computers, tablets, smartphones etc is essential for this post.

Experience of Teaching and Voluntary sector experience would be desirable for this post.

Hospital to Home Care Coordinator (H2HCC1.17)

Salary NJC Scale 6:  £23,398 :  74 Hours per fortnight

Age UK Leeds provides a range of services and products in the community, which have been specifically developed to meet the needs of older people. We are now looking for a Care Coordinator to work in the hospital.

As a Hospital to Home (H2H) Care Coordinator you will be responsible for assessing individual clients care needs and coordinating the support needed, both from our own team and other appropriate service providers.

The successful candidate will have the ability to allocate work using a 7 day rota system, develop and maintain good working relationships and ensure the service is provided consistently and to a high standard.

The ability to assess the needs of older people and the knowledge of what support is available are essential for this post.

Experience of working within a hospital environment would be of a distinct advantage.

Hospital to Home Support Worker (H2HSW1.17) X 2 Vacancies

Salary NJC Scale 4:  £18,070 per annum pro rata, 28 hours per week

Age UK Leeds provides a range of services and products in the community, which have been specifically developed to meet the needs of older people. We are now looking for TWO  Support Workers to work in the hospital.

As a  Hospital to Home (H2H) support worker (SW) you will be responsible for;

The successful candidates will have the ability to :

  • Assess individuals needs within the A & E departments , JAMA and appropriate wards and complete a structured discharge plan for eligible clients.
  • Agree with the Care Coordinator the immediate support needed and provide this accordingly. This can include transport / escorting home and practical help to assist them settling back in at home.
  • To provide , obtain and highlight sources of information and advice for older people / carers on all matters relating to their caring role , health & Social care , housing , financial matters , living with long term conditions and leisure activities .

 

The successful candidates will have experience of and skills in ;

  • Working in a similar role and at weekends , Bank Holidays and out of hours.
  • Assessing individuals care needs and providing support, including outreach support to older people, their families and carers.
  • Working with databases, inputting client data & completing client paperwork and support plans.
  • The local services available to older people, including older carers , in Leeds and especially for those who are vulnerable or socially isolated.
  • Data protection and confidentiality issues.
  • Full driving licence & access to a vehicle for business use.

Are essential for this post.

Experience of Working within a hospital environment or with people recently discharged from hospital would be desirable for this role.

You will be based at St James Hospital in Leeds.
