Care Coordinators (Social Care Team) - Reference: SCTCC1.14

Salary: NJC Scale 6 £22,443      35 Hours per week

 

We are recruiting for 2 Care Coordinators for our Social Care Team

 

As a Care Coordinator in the Social Care Team, you will be responsible for working with older people living with long term health conditions and/ or mental health issues. Together with the client you will agree and deliver a holistic assessment of their emotional, social, financial and practical needs and develop a plan to address their priority needs.

 

You will be responsible for the day to day supervision and allocation of work to a team of support workers.

 

The successful candidate will have the ability to build trust and rapport with clients and their families/ carers and the ability to develop good working relationships with GPs and other health and social care professionals. Experience of providing individual client support is essential for this post. Experience of working with older people with long term health condition and / or mental health issues would be an advantage.

 

You will be based at our city centre office in the Bradbury Building; however we expect you will spend most of your time working in the community.

Bank Support Worker- (H2HBSW1.14)

Salary : £20 a day stand by allowance, with addition of £10 for every hour worked

 

Our Hospital 2 Home project operates out of A&E at St. James Hospital, Leeds 7 days a week from 12 noon to 8pm but community support may be carried out until 10pm.

 

As a Support Worker you will be working in the community providing one to one support to vulnerable older people in their own homes. This support will include both emotional and practical support to clients.

 

Experience of providing support and working with vulnerable older people is essential for this post.

 

Successful candidates will require a clean driving licence and use of a car for work purposes. DBS and references checks will be completed for all workers.

 

You will need use of your own mobile telephone to carry out this role and costs for use of this will be reimbursed by Age UK Leeds.

Cafe Assistant - (CFAS2.14)

Salary £6.50 p/hr 3-4 shifts per week [over 6 days] occasional events and Sundays.

 

As a Café Assistant you will assist in the day to day running of the café and catering services.

 

You will maintain high standards of customer service and food presentation.

 

The successful candidate will have the ability to prapare food and hot beverages throughout the day and provide a high quality of customer service at all times.

 

Experience of working in a busy catering environment is essential for this post.

 

Experience of cash handling and a level 2 food hygiene certificate would be of a distinct advantage.

 

You will be based in the Arch Café, Bradbury Building.
