Bridge Project

The Bridge Project is a service designed to support older people (aged over 65) living in Lincolnshire, who are at risk of losing their independence.

The service has been developed in partnership with Lincolnshire County Council (LCC) and will work with individuals Bridge Projectwho are in need of secondary level support.

 

This could be following:

 

  • A critical incident in their lives
  • An episode of acute ill health (often chronic disease)
  • An assessment which indicates social isolation, poor health or gradual decline in ability to live independently

Or when time limited support is required following a hospital stay or crisis.


The service is short term, usually time limited up to a maximum of 6 weeks. It is for individuals that do not meet the LCC’s Fair Access to Care criteria but are assessed as needing secondary level support in order to maintain or re-gain improved levels of independence and prevent or reduce their need for statutory services.
