Falls Prevention Service
As we get older the risk of falls increase most taking place within the home environment. The good news there are some simple changes we can make to prevent falls. We can help you feel safer and remain independent in your own home.
About the Falls Prevention Service
- We can give falls prevention advice and support
- We can arrange a home visit to carry out a Falls Prevention Check
- The Falls team is guided by the NHS and Lincolnshire County Council
- The Project manager can take referrals from Health Professionals and the General Public
- We have four caseworkers who work in the county, they will contact a client to arrange a home visit and give support
- The Caseworker will complete a Falls Risk Assessment to identify any concerns
- We can then refer to other agencies if requested

- Age UK services
- Adult Social Care
- Handyman Services
- Telecare
- First Contact
- The Bridge Project
- The Royal British Legion
Aims of the Falls Prevention Service
- We will contact the client and arrange a home visit
- We will give advice and support to help prevent falls
- We want to help you feel safer and more independent in your own home
