We at Age UK Lincoln understand that navigating the complex health, social care and voluntary services can be challenging and at times frustrating.

The Primary Care Navigators are able to visit you either in your own home or at your GP surgery, whichever is most convenient for you. Working with you Your allocated Primary Care Navigator will talk to you about your current situation and what you want to achieve.

Together you will discuss what you want to achieve and the Primary Care Navigator will support you to access all of the services that you wish to.
