Home from Hospital
This free scheme helps vulnerable older people who need extra support following a stay in hospital.

We offer practical help and emotional support for up to six weeks following discharge from hospital.
Accompanied transport home can be provided if necessary. We can make sure you have groceries in the fridge, get prescriptions from the chemist and sit and have a chat if you need someone to talk to.
Some people are referred by via social services or by staff at the Milton Keynes and Bletchley Community Hospitals. You can also ring us directly.
There is no charge for this service.

A member of staff or a volunteer from Age UK Milton Keynes will visit you at home on a regular basis for a chat or to see that things are generally well with you. We can help fill in forms, or maybe make a telephone call that is worrying you. Call our Home visiting service to see if we can provide a bit of support. This is a free service for people over the age of 60.

We can help with shopping and light housework, and other domestic tasks that you are finding it difficult to manage, if you are over 60 and live in the borough of Milton Keynes. The current charge is £12 per hour. Find out more.
