Home from Hospital

Because coming home from hospital can be a time of worry for you and your family, as well as a time of celebration and relief.

Older people are often at their most vulnerable after an illness or health crisis, so we provide holistic assessments to establish what help is most needed and wanted — to make life a little easier.

Accident & Emergency Discharge service

Our A&E Service Organiser works closely with health and social care professionals in the multi-disciplinary team at the A&E Unit to ensure a person-centred service is provided to anyone over the age of 60 living in North Staffordshire who needs help and support after discharge from Accident and Emergency.

 

Ranging from minor to complex packages from specialist teams, our support is designed to make life a little easier, to promote wellbeing and to increase older people’s independence as far as possible.


The service organiser will visit you at home, and carry out a full assessment of your needs with you and any potential carer. It may include practical help like shopping, benefit checks and applications, or referrals to other agencies or Age UK services.

 

If needed, a trained volunteer will provide practical support for up to six weeks after discharge from Accident and Emergency.

Hospital Discharge Support service in Newcastle

Based with the Social Care and Health Discharge Team at University Hospital of North Staffordshire, this service includes doing home assessments before discharge, making follow-up calls or visits, arranging transport home, or to/from hospital or respite, and help with general shopping.

Social Rehabilitation across North Staffordshire

Our coordinators work with older people to rebuild their confidence and motivation, and support their successful return home.

 

This will follow a referral from Social Services teams in Stoke-on-Trent and Newcastle-under-Lyme, or from NHS teams in Stoke-on-Trent, Staffordshire Moorlands and Newcastle-under-Lyme.

 

This service includes:

 

  • Help with housing options (information, making applications, viewing properties, signing-up, removals)
  • Checking that the care package at home is working
  • Accessing any necessary aids and adaptations
  • Providing benefit checks and help with benefit claims and other grant applications
  • Home safety and security advice and help
  • Making referrals to other agencies and services as well as to other Age UK services
  • Offering other practical, social and emotional support.
