Social Prescribing Coordinator

Salary - Band 23-26 (£20,561– £22,616)

PURPOSE:

The Social Prescribing Service is a partnership initiative, which aims to improve people’s health and wellbeing by linking them with activities in the community and increasing their independence.

This role involves working with people who are socially isolated, or who experience other barriers to engaging in the community, to help them achieve goals to support them to become more independent. This may include providing them with access to a package of support, including groups and activities, one to one support or volunteer befriending.

REQUIREMENTS:

  • NVQ Level 3 Health and Social Care desirable
  • Experience of working within Health & Social Care
  • Experience of working with and supporting volunteers is desirable
  • Experience of older peoples’ issues and mental health issues is desirable

Older Offenders Researcher

Salary – Band 23-26, £20,651-£22,616, pro rata

Age UK North Tyneside has been working for 3 years to support older offenders across the North East and to improve the range of services and support available to them. We now have an exciting opportunity to develop an Older People’s Strategy for the prisons across the North East and Cumbria. This Strategy will be pivotal in influencing the Older Offender’s agenda across the region. This role will be key to the delivery and development of this strategy.

PURPOSE : To support the Head of Community Services to develop an Older People’s Strategy for Prisons across the North East and Cumbria. To support the delivery of the strategy project plan including conducting needs and stakeholder analysis, conducting consultation with prisoners, supporting the report writing and finally supporting the dissemination of the strategy.

REQUIREMENTS:

  • Masters in relevant discipline (desirable)
  • Good honours degree in relevant discipline (essential)
  • Criminal Justice related qualifications (desirable)
  • Research qualification (desirable)

Carer with Driving Responsibilities

  • Hours: Part time (16 hours per week)
  • Salary: £7.73 per hour

Age Uk North Tyneside are looking for an experienced Carer with Driving Responsibilities to come and work as part of the Wellbeing Centers. This is an exciting opportunity for the successful candidate to be able to develop their skill set further. This is a highly responsible position which listens and responds to the needs and aspirations of individual customers and carers within a Dementia centre.

To share with other staff in meeting the personal care needs of Wellbeing Centre users, in a way that respects dignity of the individual and promotes independence.

To help maintain the physical environment of the centre and to assist with the general day to day activities in a way that meets the needs and interests of the customers.

Successful Candidates will be required to:

  • To drive and escort one of Age UK North Tyneside (AUKNT) vehicles on a daily basis in accordance with AUKNT policies and procedures
  • To ensure the care and safety of customers during journeys
  • To help customers, by using planned care, with all personal care tasks, mobility problems and assisting in the use of care aids
  • Assist in caring for customers that are temporarily sick and needing, for example changing of a minor dressing, or escorting to hospital
  • Help in the promotion of cognitive and physical activities for customers through outings, conversation and other activities such as reading, writing, hobbies and recreational interests, reminiscence
  • To assist in setting tables and trays; serving meals; supporting customers over meal times, preparing light meals; supporting customers to washing up and undertaking light cleaning duties as required
  • To read and record accurate daily activities/wellbeing of customers in their personal files, take part in staff and other training and meetings as required
  • To observe customers’ personal and organisational confidentiality at all levels
  • To follow the daily transport route plan
  • To participate in training courses and activities as required

To offer practical, flexible assistance to help people aged 50 and over to remain independent, gain confidence and provide support to maintain social networks in accordance with agreed ‘care and support plans’. Customer’s may be resident in their own homes in the North Tyneside area or may be resident at one of the Extra Care Housing Schemes operated by EveryDay, again in the North Tyneside vicinity.

To work with other staff members in meeting the physical, emotional and social needs of tenants and customers, in a way that promotes independence and respects the dignity of individuals. After successful completion of intensive two week paid induction programme.
