Hospital discharge scheme

The hospital discharge scheme has been operating since February 2007 and assists in the reduction of people being readmitted to hospital.

The scheme aims to promote a good quality of life though supporting reablement giving choice and control of independence for all older people in Sunderland.

How do I qualify for the hospital discharge scheme?

  • If you are over 60
  • Live in Sunderland
  • Are without a Care Plan

The scheme aims to promote a good quality of life though supporting reablement giving choice and control of independence for all older people in Sunderland.

 

What can we do for you?

Assist with

  • Shopping
  • Light housework
  • Cook
  • Collect prescriptions
  • Companionship

 

Our free service can provide upto 5 visits following discharge from hospital at a time convenient to you. We are all individual and may need different levels of support.

 

We can make visits 5 days a week between the hours of 9am to 6pm.
