Following an internal promotion, Age UK Sutton is seeking a new Senior Manager to lead their pioneering Home from Hospital (HFH) service in the London Borough of Sutton, which provides practical support on discharge from hospital.
HFH is a service funded by the NHS through the Sutton Clinical Commissioning Group. The service supports older people (65+) in Sutton in order to:
Reduce admissions to accident and emergency departments
Reduce unnecessary hospital admissions
Reduce number of readmissions to hospital
Increase motivation, confidence and social integration after a hospital stay
Age UK Sutton works closely with partners including GPs, pharmacists, hospital staff and social services to ensure that we work collaboratively to deliver patient centred services.
You will have a proven record of leadership and project/programme management, with experience of working with health and social care professionals. You will understand assessment practices and be adept at outcomes focused monitoring and evidence based practice. Empathy for and understanding of vulnerable older people and of the contribution they can make to society is a must.
