Advancing Illness and End of Life Support Network
Staff specifically trained in end of life care will support isolated, older people (aged 50+) who have little or no informal care network (Family/friends). Individuals must live in Tower Hamlets and be aware of their prognosis.
This service offers the following support:
- Befriending
- Practical (i.e. dealing with correspondence, booking appointments)
- Escorting to places of interest or appointments
- Signposting to appropriate services, following up referrals etc.
Individuals can access up to 14 hours support per week (depending on a needs assessment and on-going reviews).
