 30 hours per week Tues - Fri

Salary:

£22,717pa

Location:

Home Working with 1 day per week at a designated ICN Hub in Bromley  

Closing Date:

 midnight 7th January 2024

Do you have experience in health and social care or third sector information and advice? Have you helped to deliver a service to people with complex needs? Are you comfortable working with minimal supervision?

The Care Navigator will work with colleagues from health and social care to develop a multi-disciplinary approach to patient care, ensuring a holistic approach is taken.
