Transport home and support after a stay in hospital.

Home from Hospital Seamless Service

We are working in partnership with Community Transport in Calderdale and Kirklees to provide this service.
We offer a seamless transport and support service providing a safe supported journey home by Community Transport and patients are met by an Age UK staff member or volunteer to provide immediate support at home.

🛈 How to book the service.
The seamless service runs Mon-Fri 10.30am to 8.30pm and weekends 11am to 6pm

You must book this service through Hospital staff in Calderdale and Kirklees: contact the coordinators on 0300 0200213 who are available Monday - Sunday, 10 am til 5.30 pm.

What support do we provide?

We take weight bearing clients from the age of 50 + home from hospital in Calderdale & Kirklees.

We spend time settling clients back in at home making sure they have food, heating and electricity but most importantly we make sure they are safe and secure in their home before we leave them.

We speak to clients and spend time in the home looking at other services which may be of use to them - for example, our befriending service, as they can feel lonely after a long stay in hospital surrounded by people and then suddenly back home all alone; we also look at other services including our cleaning service, wellbeing centres and dementia support.

Six Week Home From Hospital Service—Calderdale

This is a personalised package to meet the individual's (and their carer's) needs and focuses on re-engagement and maintaining independence. The service can include any combination of the following for up to 6 weeks following hospital discharge:

- Settling in service which includes the following checks: environmental falls assessment, safety and wellbeing checks
- Key safe and handrail fitting (excludes cost of equipment)
- Regular telephone welfare calls
- Weekly home visit
- Assisted visits to local community groups
- Prescription collection
- Taster day at Age UK Wellbeing Centre (includes transport, hot meal, companionship, and a range of activities)
- Information & Advice including benefits checks
