Eating Well: Making sure people have access to food at hospital discharge and beyond.

Our Hospital Discharge and Admission Avoidance Support Service, ensures that (amongst many other things) people have access to food when they are discharged from hospital. We recently took part in a project with other voluntary organisations who want to make certain that patients can access appetising and nutritional meals and/or food after being discharged from hospital to assist with their recovery and reduce readmission. 

We contributed to the work being done by Sustain (an alliance of organisations and communities who work together for a better system of food, farming and fishing), taking part in an online awareness webinar and publication. 

Sustain has published a new guide called Home from hospital: ensuring people have access to food at hospital discharge and beyond, with a foreword by Sustain patron Dame Prue Leith.

" We have now published our new guide: Home from hospital: ensuring people have access to food at hospital discharge and beyond. Many thanks indeed to all those who contributed to the project and shared their vitally important work. 

This guide summarises five key elements of ensuring people have support in place and includes just some examples of good practice from around the country. Download the guide and watch the recording of the launch event, featuring speakers from Edinburgh, Blackburn with Darwen, Cambridgeshire and Peterborough. The project was kindly supported by the Churchill Fellowship and the Rank Foundation.
