The Ceredigion 3rd Sector Community Resource Team pilot project provides clients who have been affected by poor health with a six week series of wrap-around intensive targeted interventions.

These interventions aim to improve the client's quality of life and prevent hospital re-admissions by providing assistance in the home after hospital discharge, and information and advice to ensure that housing conditions are optimal, income is maximised through access to benefits, and that all available practical support needed is in place. The service also includes the provision of home adaptations and, where available, offers a befriending service. By improving a person's home circumstances, many of the stressors on their health are avoided.

The project partners – Age Cymru Dyfed, Citizens' Advice and Care & Repair – are funded under the Intermediate Care Fund (ICF), led by Hywel Dda Health Board and Ceredigion County Council's Social Service department, and administered by CAVO, the Ceredigion CVC.

The partners have spent the first two quarters of the pilot supporting some 140 clients, continually defining the project and tweaking it so as to produce a design that, if it is taken forward after the end of the ICF funding period in March 2018, will be able to deal with the large volume of referrals that would be expected from Health / Social Services professionals located within Porth Gofal (the Call Centre), GP surgeries, hospitals, Social Services, and other teams in the county providing social and medical support.

Initial referrals are taken by British Red Cross (BRC), who then contact appropriate project partners according to the client's needs.
