iCan - supporting adults over 18 to live independently

Our iCan service support adults 18+ who may be facing social or economic challenges, have a long-term health condition and are isolated, to improve their health and wellbeing, enabling people to remain independent, and connect with others and their community. It is a community navigation service led by Age UK Enfield, with Alpha Cares, Attend and Enfield Carers Centre. We are funded by London Borough of Enfield.
The iCan navigators are based in 4 geoographic areas across Enfield, where they work one to one with residents, supporting them to find solutions and make improvements to their health and wellbeing.
You can refer yourself to the service, or you can be referred by a family member or friend, or a professional. One of our navigators will contact you to arrange an initial conversation, where you can discuss your needs and what you would like to achieve. This will form the basis of an action plan that you and your navigator can work towards.
Our navigators can help in a number of ways:
Help to access information and advice (including benefits or housing advice)
Help to access to health and social care services
Provide advice on lifestyle, or healthy living choices
Help you to meet new people or reconnecting with their community
Build skills and confidence or volunteering
Help to find places to go and things to do
Help to understand and self-manage your health conditions
We also deliver monthly group sessions, including:
Peer support groups - Stroke café weekly , Diabetes Group monthly, Walks& Talk monthly
Health information and awareness sessions
Social activity groups – Tea and Chatter sessions in Enfield Town Library, Edmonton Green Library and Ordanance Road Library
