Our Young Onset Dementia Support Worker can provide countywide support to local people under 65 living with dementia or cognitive impairment, including those who may also have a learning disability. Working with the Hertswise partnership, your Support Worker can help with:
Information and Advice
A service to help you explore your rights and entitlements. We can provide support in claiming benefits, Lasting Powers of Attorney, guidance around dementia, accessing social activities, and community and healthcare services.
Activities based on the needs of the attendees, including community activities to improve mental and physical wellbeing. This could include; walking, board games, arts and crafts, crazy golf, bird watching, gardening, and much more!
One to One Activities
If you don’t feel confident about joining a group but would still like support within the community, our young onset dementia worker can offer home visits. We will support you in gaining confidence and accessing new services in Hertfordshire and we can also help you to maintain any current interests or hobbies.
One to one support can be offered to anyone, even if you are already attending the groups or local Hertswise Hubs.
