Responsible to: Carers IW Link Worker
Accountable to: Carers IW 
Contract Type: 20 months fixed term contract
To establish an effective service to identify and support Carers within Isle of Wight & Sevenacres Hospital and to work with hospital staff to ensure that they take carers properly into account in patient care. The team will oversee the service of the Carers Lounge coordinating volunteers for the maximum benefit of visiting carers. 
The purpose of the Living Well Carer Service is to put unpaid carers in personal control of their health & wellbeing and provide them with integrated care and support in their local community, focusing on prevention of ill health and the promotion of wellbeing. Supporting the carer’s involvement in everyday care for the patient, discharge planning with a strong re-enablement focus so to promote early discharge. 
The Living Well Carer Service will enable unpaid carers to have improved health outcomes and independence whilst reducing the burden on NHS services on the Island through the reduction in need for GP visits, Out of Hours Contacts, emergency admissions/readmissions etc.

The Living Well approach:

  • Promotes people’s health and wellbeing, supporting them to increase their ability to live well and retain their independence
  • Supports unpaid carers in the hospital setting
  • Builds new partnerships to support the delivery of person-centred, holistic care planning and co-ordination
  • Works with Adult Social Care to support those who feel on the edge of crisis, particularly those not eligible for statutory funding
  • Supports people leaving hospital by providing new models of care in the community

The Carers IW Living Well Team is looking to recruit the following:

Living Well Carer Support Worker (£20,500 pro rata for 18 hours per week)

This a job share opportunity to work in the Carers Lounge within St Mary’s & Sevenacres Hospital to identify and support unpaid carers within the hospital setting for 2.5 days per week on a fixed term contract (May 2019).
