About the role

Age United in partnership with Age UK Kensington and Chelsea is delighted to be working together with West London CCG and Kensington and Chelsea Social Council to offer local residents innovative support to address their wider health and social care needs.

We have three new Social Prescribing Link Worker posts available for people who are passionate about connecting communities with the tailored support they need to live independent and healthier lives, based within Kensington and Chelsea’s new Primary Care Networks. Link Workers will provide connections between the GP and community services that can support residents to address their wider health and care needs.

We also have a new North Kensington Social Prescribing Link Worker post which will be based across that area, supporting a new programme being funded by West London CCG.

The Link Worker programme is a national initiative from NHS England and NHS Improvement, and we are seeking individuals with a background or interest in health, care, public health or voluntary sector services.

We are happy to discuss secondments from Third Sector organisations within Kensington and Chelsea. Please contact Lin Gillians, the Interim Service Manager: lgillians@aukc.org.uk

Full training is provided.
