Location: Royal Lancaster Infirmary.

Salary: Scale 4 pt 20 £18637pa pro rata. 

Hours: 24.5hrs per week. Working between the hours of 9am-7pm on a rota basis including weekends and bank holidays.

Age UK Lancashire is the leader in delivering Hospital Aftercare Services across Lancashire. We now have an exciting opportunity to expand the delivery of this service.

The current pressure on NHS hospitals to discharge patients as quickly as possible is considerable. Age UK Lancashire plays a key role in supporting this work and we require a Hospital Aftercare Allocation Officer to be based in Royal Lancaster Infirmary. The role is to support workers who transport and settle patients back into their home after discharge from Hospital. There is an extended period of support and the person in this role will ensure the smooth running of this support, administration and appropriate signposting to other services. 

The successful applicant must have:  

  • Experience of supervising staff.        
  • An awareness of the needs and issues affecting vulnerable or older people.
  • Experience of using I.T systems and quality assuring data. 
  •  Experience of signposting to relevant agencies and organisations.
  •  Experience of effective time management.

There is no personal care involved in this role.  

This post has been funded initially until 30/04/2020.

The nature of this post will require travel across the area. A full driving licence appropriate insurance cover and use of a suitable car is essential.
