Location: Royal Preston Chorley & South Ribble Hospitals
Term: 6 months contract with a possibility of extension
Salary: £9.90ph £18,018pa
Hours: Full time - 35hrs per week, working between the hours of 9am-7pm on a rota basis including bank holidays.
Due to the Covid 19 pandemic, the current pressures on NHS hospitals to discharge patients as quickly as possible is considerable. Age UK Lancashire plays a key role in supporting this work and we require a Hospital Aftercare Wellbeing worker to be based in Preston and Chorley General Hospitals. This role is to support patients across Central Lancashire by taking them home following their discharge from hospital and ensuring they are safely settled back in their homes. We are looking for someone with:
- Experience of working with vulnerable people
- Experience of providing practical and emotional support
- Experience of signposting to relevant agencies and organisations
- Experience of managing your own workload and keeping accurate records and assessments
This is a hugely rewarding role. You will be working as a part of a team that provides much needed help and support to older people in the community after being discharged from hospital.
The nature of the post will require travel across the area. A full driving licence, appropriate insurance cover and use of a suitable car is essential.
(There is no personal care involved in this role)
