The Service is available to support any individual who is in need of support for light household chores. The service is to enable the individual back to their normal independence following on from a Holistic Assessment. There is an initial goal setting that is agreed at the start of our involvement that ensures that adequate support is agreed over a short time span of up to 6 weeks.

The Hospital Aftercare Service has been in operation since 2000. The aim of our Service is to assist individuals back to their normal independence, following a hospital discharge, period of illness or injury. We aim to help you regain your independence and provide low level practical and emotional support.

Eligibility Criteria: You must be aged 18+ and your GP must be registered in the local district The service runs 7 days a week and the times are variable to suit the clients needs
