About the role

Live Life is a Social Prescribing service working with all GP Practices in Milton Keynes, with the aim of improving our clients' wellbeing and reducing the number of unnecessary visits to doctors in the practice.

We are recruiting someone to work exclusively with the GP practices in Ashfield Medical Centre, Central Milton Keynes Medical Centre, Fishermead Medical Centre, Milton Keynes Village Practice, The Grove Medical Centre and Walnut Tree Health Centre to develop and deliver a Social Prescribing service for their patients.  You will be responsible for the day-to-day management of a caseload of clients who need help and support to manage issues in their lives which are having a negative impact on their general health and wellbeing.  You will organise referrals and client appointments, including visiting people in their own home and identifying services that will help them live independently.  You will take a person centred approach to supporting and motivating clients to achieve their goals and, where appropriate, introduce clients to clubs and activities.

This post involved direct contact with people age 18+ in their own home, at Age UK Milton Keynes premises, in all the GP practices in The East:MK Network and in the community. 

About you

You must have an understanding of, and empathy with, vulnerable adults and the ability to communicate effectively with all types of people.  Good listening skills and being able to put people at their ease and gain their trust are essential.  You must be able to plan and prioritise work and adapt to new models of working. 

The ability to use Windows package, particularly Word, Outlook and Excel, will be needed for this role and you must have a telephone and access to the internet.  We are looking for someone with a good knowledge of Milton Keynes who has excellent record keeping skills and the ability to write up case studies.
