Age UK Norfolk’s pilot providing free non-medical information and advice in GP Surgeries in South Norfolk for people over 65 and their carers will continue in 3 of the existing surgeries until the end of the year.
NHS South Norfolk Clinical Commissioning Group and Norfolk County Council are funding the service, as part of their “Better Care for Norfolk” and “Strong and Well” programmes.
Whilst the project was funded up to March 2016, it has been decided to continue until the end of the year in 3 of the most successful surgeries: Harleston, Wymondham and Watton. In the meantime the pilot’s evaluation is underway and will be available soon.
In addition Age UK Norfolk will be providing this service at the School Lane Surgery in Thetford every Wednesday morning.
GPs and practice nurses are able to refer patients to the service, or patients can book their own appointment without referral if they wish. Our trained advisers, working with partner organisations, are available to provide practical help and support on a wide range of later life issues, including social care, benefits, Dementia support, befriending, caring for older relatives, help at home, housing options and Lasting Power of Attorney.
Sessions take place once a week. Please contact your surgery to make an appointment.
