About the role
We are looking for someone to join our Collaborative Care Team that provides practical and emotional support to vulnerable older people, helping them to avoid hospital admissions or the need to move into residential care. By supporting people with day-to-day tasks such as shopping for groceries, making doctor's appointments and generally keeping well, you can help people maintain their independence and enjoy living in their own home.
You must have a good standard of general education and have experience of working with older people. We need someone with the ability to work flexibly and with access to a car. This is a job which can be very satisfying, where you can have a huge positive effect on a person's wellbeing by using your initiative and being empathetic. You will be confident in making telephone calls and be good at problem solving and working collaboratively with other organisations.
