Location:   Covering GP surgeries in Kettering area, based at our Irthlingborough office

Rate of pay:  up to £22,857

Hours of work:  up to 37 hours per week

About the role

We are looking for a caring, confident person to work in a task centred way, primarily with older people who are at risk of hospitalisation or becoming dependent on statutory and/or institutional care.  Age Well gives older people the support they need to look after their own health and wellbeing.  With our support people who have one or more long-term health conditions can regain their confidence and have more control over their lives. 

You will take referrals from GP surgeries in the Kettering area, visiting clients in their own homes to carry out a holistic assessment to see what course of action would have a positive effect on their wellbeing.  You will gather information so that benefit checks can be completed to maximise our clients' income and help increase their independence and options.

Our Age Well Coordinators enable clients and their family to participate in multi-disciplinary meetings and extended patient reviews with health professionals so that services and support can be put into place with their agreement.  This can be a challenging role where no two days are the same.  You will be encouraged to use your own initiative to solve problems and must be confident meeting people from all walks of life.  In return, you will be rewarded by seeing the positive difference you are making to clients and achieve a high level of job satisfaction.

In consultation with the Wellbeing Integration Lead our Age Well Coordinator will play an active role in their own personal development and the growth of the Age Well Team.  They are also responsible for entering details on a local computer system and subsequently ensuring that all records are accurate and up-to-date.

About you

We are looking for someone educated to A level or equivalent with experience of working with older people and who can demonstrate the ability to work in a non-judgemental manner.  You must be able to listen to others and absorb information as well as be able to instil motivation in others. 

You will need to plan ahead and work in a systematic and organised way.  If you are motivated by challenge and enjoy seeing problems through to their solution, knowing that your timely actions have improved life for someone else, this is the job for you.  If you thrive on being flexible and meeting new people, we would love to hear from you.  

A good knowledge of services available to older people in Northamptonshire is desirable. You must be a car driver with access to your own vehicle.
