About the role
We need a caring, confident person to join our small team that provides support to older people who are at risk of hospitalisation or becoming dependent on statutory and/or institutional care. You will liaise with the Hospital Discharge & Community Team Coordinator who will delegate specific tasks such as telephone calls to check if a client has any concerns or needs. You will accompany clients to GP or hospital appointments, collect or arrange the delivery of prescriptions and shopping, for example. You may also carry out light cleaning tasks in the home and encourage clients to establish an exercise routine. You may be asked to attend multi-disciplinary meetings at GP surgeries from time to time. The aim of this role is to help older people carry on living independently and avoid unnecessary admissions to hospital. Sometimes making a telephone call or giving someone the confidence to join a weekly Tai Chi class can turn their life around.
We are looking for a reliable person who can use their initiative to problem-solve and liaise with outside agencies on behalf of our clients. You might have a background in care work but no longer wish to work weekends or evening shifts, for example. Or perhaps you just want a change of career and would like a role with more job satisfaction, knowing that you are making a positive difference. Perhaps you have always worked in an office and would now prefer to be more active out in the community.
If you have the confidence to work alone but also enjoy being part of a team, this role would suit you. You must have empathy for older, vulnerable people and an understanding of the behaviour of others and the contributory factors behind it. No two days will be the same, as the people you help will all have their own needs and personalities. You will work as directed by the Coordinator, providing consistent support that, week to week, can improve a person's wellbeing and help them make more of life. We need someone with excellent interpersonal skills and the ability to record accurately, both verbally and in writing. A good knowledge of services available to older people in Northamptonshire would be useful and experience of care work is desirable but not essential. Car driver and access to a car is essential as you will be travelling around the county, visiting people at home in the Northampton and Brackley areas.
