NHS England Surge supports older people at local hospitals

Our small team aims to help speed up the discharge process for vulnerable patients and also prevent unnecessary re-admissions to hospital.  We work closely with Kettering General Hospital and Northampton General Hospital hospital discharge co-ordinators, other health care professional and social services, helping to ensure that everything is in place to enable patients to go home safely.

  • Welfare calls and visits

ICAN Call Back at Kettering General Hospital

Our team of callers will ask:

• How are you feeling?
• Were you admitted to KGH due to a fall?
• Are you managing your daily living tasks – washing and dressing?
• What Package of Care have you been sent home with?
• Do you have family or friends support?
• Do you need support with duties around the house etc?

We aim to contact all patients age 55 or over within 48 hours of them being discharged from Kettering General Hospital.  We will call them to find out how they they are managing once they have had time to settle at home. 

We check to see if we can be of any assistance, referring people on to our Telephone Befriending Service or Handyperson Service, or maybe arranging for our Information & Advice team to carry out a benefit assessment.  A change in a person's health or mobility could mean that they are interested in personal alarms or stairlifts, for example.  We also refer patients back to the hospital if there are any discharge concerns.
