Our team provides practical and emotional support to frail and older patients whilst they are in A&E and the Discharge Lounge.

Our team is based at Northampton General Hospital

 

We provide practical and emotional support that helps to prevent unnecessary admissions to hospital and also help speed up the discharge process for more vulnerable patients.

The team is based in Northampton General Hospital seven days a week.

We can help in A&E with

  • contacting relatives and carers for the patient
  • helping patients with eating and drinking
  • escorting patients to other departments
  • supporting patients with practical and emotional needs
  • liaising with appropriate teams to organise discharge arrangements identified by A&E staff

We can help in the hospital with

  • making a brief record of events during each shift
  • reporting untoward events or incidents to Trust staff
  • escorting patients to other departments
  • supporting patients with practical and emotional needs
  • supporting Trust staff to maintain patients' privacy and dignity

Going home

  • we supply information leaflets
  • we work with hospital staff to enable patients to go home in a timely manner
  • we can accompany patients home where appropriate
  • we can collect medications and prescriptions
  • we provide other practical support in the home that will enable the patient to retun home safely, such as making telephone calls and other tasks

We are not able to escort patients to the toilet, supervise patients on behalf of nursing staff or carry out personal care.
