When it comes to leaving hospital, it's not uncommon to find yourself or your family feeling a little unsure about how you'll manage back home. The free Home from Hospital Service by Age UK North Craven aims to assist your safe, successful and timely return home.

The service is available to those coming home from any hospital setting who:

  • Are aged 18 and over.
  • Live in North Yorkshire.
  • Are not in receipt of a Social Services care package. (Excluding North Yorkshire County Council's Independence Team).
  • Don't have complex long -term care needs.

What does the service cost?

There is no charge for the core service. A mileage rate is charged for any additional journeys - e.g. a separate shopping trip.

How can we help?

We have a dedicated team of staff and volunteers trained to provide you with reassurance and practical help. This can be offered over a period of up to 6 weeks, depending on our assessment of your needs.

Our visits are usually up to 1 hour and weekly although the frequency can be varied to best suit your circumstances.

Examples of how we help include:

  • Doing shopping (apoogies we can't buy alcohol or cigarettes for you).
  • Collecting prescriptions.
  • Making a snack.
  • Light essential household tasks.
  • Companionship and progress check.
  • Support in rebuilding confidence and independence.
  • Liaison with health and social care teams.
  • (Re)-connection or signposting to local support and/or social groups.

We don't provide personal care (e.g. assist with dressing, toileting, washing), administer medicines or provide medical care.

How can I access this service?

A member of your hospital care team should refer you. Let the staff know you're interested in the service or ring us ahead of any pre-planned admission.

On discharge, our worker will visit to agree how we can support you or will take instruction from the hospital team.
