Have you, or someone you know received care in your home from any health and social care services, since 1st March 2013? If so, the Public Health Agency would like to hear about your experience.

The 10,000 voices project aims to capture the stories of patients across a range of health and social care services and use this information to inform how future services are developed.

You can participate by completing a short survey online but if you need any help or would like an alternative version made available, you can contact the project team on the number below and they will assist you.

Age NI is encouraging everyone with experience of these services to take part and responses are welcome from patients, as well as their families and carers. You won’t be asked for any personal information, just your experience.

Responses to the survey are gathered on an ongoing basis and the insights gained are used by the Public Health Agency to help inform important issues such as the future planning and funding of health and social care services.
