Are you living with or beyond cancer?
If so, we are here to help.
It's a cause for celebration that more people than ever are surviving for longer after a cancer diagnosis, but we know the impact of cancer doesn't suddenly stop during treatment and when treatment is over.
We want everyone who is diagnosed with cancer to live well after a diagnosis and believe everyone should experience high quality cancer care and support. If you have a cancer diagnosis, a member of our Integrated Care Team can visit you in your home or community setting. They can support you to create a plan to help you feel more in control of your health and wellbeing.
This service offers people who have been diagnosed, are receiving treatment or have recovered from cancer, the opportunity to have a needs assessment to ensure their physical, emotional and financial needs are being met.
Age UK North Tyneside is working with The Northern Cancer Alliance to help improve the experience of people living with and beyond cancer.
Working one-to-one with a key worker, a care plan will be devised to address any concerns the customer may have and link with voluntary organisations to address those needs.
The service covers:
- Practical concerns
- Spiritual and religious concerns
- Lifestyle and information
- Physical concerns
- Family and relationship concerns
- Emotional concerns
