Health and Community Support Lead

We have an exciting opportunity for a creative, inspired, resourceful and team playing individual to join our successful Health and Community Support team, to provide support to some of our most vulnerable, lonely and isolated clients through our Independent Living service.

The Health and Community Support team provide essential help in a variety of ways to the ageing population of Norwich, and in particular some of the more vulnerable people. This may be through our recovery and support service, health @ home service, befriending, groups and activities, and our pilots that are in conjunction with other leading partners.

You will feed into the Health and Community Support Manager, by providing reports on the successes, challenges, and finances, as well as spending some time in the field helping to promote the groups and activities to reach as many people as possible.

This post will challenge your creativity and afford you the opportunity to work collaboratively with external partners including statutory and non-statutory organisations. You will support and coach volunteers to inspire customers to build resilience and engage in their local community.

We seek someone with a track record of working with older people in the community or a social care setting, with a solid understanding of health conditions that many older people live with, and the risks and care required to best manage these.

You will monitor and evaluate the service regularly through surveys, questionnaires and 1-2-1 contact with customers in line with funding recruitments.

You will be working alongside our current Community Outreach Officers who run our community groups/activities and Community Support Worker who delivers our Health @ Home service. You will be a good team player, possess excellent communication skills and have a flexible approach to working hours/days.

£24,000.00 (Full Time Equivalent) + contributory pension scheme + health scheme. Salary will be pro-rata to hours worked.

Location – Norwich, Mix between home working, in the community and at the Marion Road Centre.

37 hours/week, flexible working pattern.
