CASE STUDY: Living well with dementia

Kate's case studyWhen "Kate" was discharged from hospital, her memory loss meant she was highly likely to be readmitted. But we helped her to put a support network in place to enable Kate to live well at home and minimise the likelihood of a future readmission.

 

Our support to Kate is part of our free Independent Living Co-ordination service, which enables people aged 50 and over to live well at home, minimising the impact on local health and social care services.
