Service Navigation is a person-centred service that is delivered through home visits and by telephone. We work closely with individuals to identify needs and service requirements, providing necessary support and sign posting to other organisations that meet the needs more appropriately.
What can you expect?
Following referral to Age UK Southampton you will be visited at home at a mutually convenient date and time. The Service Navigator will discuss with you how things are going presently and what support you currently have, how you manage your day, what you might like to change and your personal goals for the future. The conversation is about you and is a person centred assessment. It is intended to help you to make choices that will improve your daily living, maintain you independence, and improve your general health and wellbeing.
After the visit the Service Navigator will record the details of your conversation. Everything that you have told us will be treated with the strictest confidence. They will then signpost you to any of the services either internally as provided by Age UK Southampton or externally that you have identified. You will be supported to access the relevant service and any information that you need will be sent to you.
If you have asked to be visited by one of our Volunteer Visiting or Telephone befrienders and you are later matched with a volunteer the situation will be closely monitored. The Service Navigator will contact you to arrange a telephone and later home visit reviews. Any new needs will be identified.
At all times you will be treated with respect and your personal views, needs and desires will be fully taken into account.
