Welcome Home aims to provide support and company for those older people who have been discharged from a hospital stay and live alone in Redcar and Cleveland.

Before or upon discharge suitable patients will be matched with a project volunteer who will support the patient to get home and will offer 4-6 weekly visits (1 visit per week) at the older persons home to help build their wellbeing and independence and reduce re-admission rates

Responsibilities

To provide support upon return to home following a hospital stay

To ensure that the patient has the necessary basic needs (gas, electricity and food) available upon discharge

To provide 4-6 weekly (1 per week) befriending visits following discharge and provide company and where necessary signposting to other appropriate services

To report any concerns to line manager

Undertake any necessary training required for this role

 

Skills and Knowledge

Volunteers should be reliable, non-judgmental and have good communication skills.  A basic knowledge is issues affecting older lonely people who have been discharged from hospital is desirable.

The ability to build good rapport with patients and to work alone is also required

 

Additional Information

All volunteers will be subject to an enhanced level DBS check.

All volunteers must commit to a minimum of 2 hours per week for a maximum 6 week period at a time

All volunteers will be supported and supervised.

Travel expenses and all out of pocket expenses will be reimbursed
