- socially isolated
- lacking social and community support
- in need of information or advocacy
How the programme works...
Our Support Worker will visit you in your home, on a one-to-one basis, to make an assesment of your needs and provide appropriate support which could be:
- practical advice
- emotional support
- financial advice
