Age UK Wandsworth is offering a pilot Outreach Service for people in Wandsworth who have been diagnosed with dementia, adults with disabilities and those over 65. We are also able to support anyone caring for those clients. Referrals are through Wandsworth Adult Social Services and Age UK Wandsworth will work with Adult Social Services to ensure the client’s support plan and goals are being met.

The Outreach Service can deliver a wide range of activities and companionship to meet the client’s needs and to help them gain confidence, pursue their interests, and access local services. The Outreach team can also provide respite for carers. Duration of the service will be based on assessed need and range from minimum of 1 hour to a maximum of 5 hours per session, per week. Sessions can be delivered face to face, over the telephone, digitally or a combination that suits the client’s needs.

When personal care is required as part of an individual’s support plan, Age UK Wandsworth will enlist the help of a local care agency to deliver low needs personal care, in addition to the activities and companionship.

All of these services are available to carers who need respite but are worried about leaving their loved one without assistance.

In addition to regular Outreach visits, Age UK Wandsworth can support those in our local community with digital support. If an Outreach client would like to have one-on-one tutorials about their devices, a Digital Outreach Assistant will schedule an initial visit and follow up sessions to combat social isolation and digital exclusion.
