                                 22 hours (Brighton & Hove) - £14,698 per annum (£24,720 FTE)

                                28-37 hours (Bognor area, Arun) £18,707 - £24,720 per annum

About The Role

We are looking for a compassionate and proactive Social Prescriber to join our growing team!  A Social Prescriber connects individuals with non-medical sources of support and resources within their community to improve their wellbeing.  With the role funded by the local Primary Care Network, you’ll be part of a multi-disciplinary team in a GP surgery and working as a part of a fabulous community partnership. 

The role is to provide help to patients by sharing knowledge and expertise about the local area and how to access it.  You will be connecting them to community groups and services for practical and emotional support.  The short-term support you give will focus on what matters to them, taking a holistic and person-centred approach. 

Although our organisation supports people over the age of 50, our Social Prescribers support patients over 18. You will be helping them to set goals, encourage behaviour change, build resilience, increase social connectivity, and help enable people to have greater control over their health and lives.

Please note, there may be a requirement to provide status of and have further vaccinations in line with NHS recruitment guidelines.

What you will need to be successful in the role

  • An understanding of the principles of Social Prescribing and the social determinates of health.
  • Excellent communication skills.
  • Ability to build rapport quickly with patients, peers, and colleagues across different organisations.
  • Expertise in working 121 with patients by actively listening and using techniques to hold and frame conversations.
  • Solid professional boundaries and personal resilience in supporting patients.
  • Sound digital and time-management skills to work effectively and efficiently.
  • Ability to write appropriate case-notes on two systems, keep accurate records as well as managing own diary.
  • Be comfortable, confident, and safe working remotely in a GP surgery, home or in an office.
  • Be an active part of the wider-team and share knowledge in developing the service.
  • Familiarity with health and social care systems and local community services would be advantageous.
  • Driving licence and use of a car.
