About this role

Age UK Wiltshire is seeking to recruit an enthusiastic, proactive and motivated person to join our new Home from Hospital project. The successful candidate will be based at the Age UK Wiltshire office in Salisbury.

This is an exciting opportunity to work as part of a team to help and support individuals aged 65 and above who have been identified through the discharge process to help them regain their confidence and independent living skills using a combination of practical and emotional support for a short period of time (up to four weeks).

Age UK Wiltshire anticipates that the Home from Hospital project will improve the wellbeing of older people, improve their general health and help prevent a re-admission to hospital. The project provides non-medical practical daily living support to individuals and may include troubleshooting immediate needs such as resolving medication confusion, accessing equipment etc and/or giving immediate practical help such as essential shopping or light housework.

As well as having the ability to engage with older people effectively this role requires someone who is a self-starter and able to work on their own initiative. The successful candidate will have energy and enthusiasm and an ability to monitor and develop the project. A positive approach to team work is essential as a large proportion of the post will be assigning Age UK Wiltshire Volunteers to give the individual an agreed level of support. The post will work with, support and supervise those volunteers but equally be prepared to give the required help to individuals when necessary and appropriate.

The nature of this post will require travel within a specified area therefore a requirement of this post will be access to and the use of a reliable car and a current driving licence.

This role will be subject to a Disclosure and Barring Service check.
