£22,500 PA pro rata for 21 hours per week

21hrs a week, Monday - Wednesday (flexible)

About this role

Age UK Wiltshire is seeking to recruit an enthusiastic, proactive and motivated person to join our Living Well project. The successful candidate will be based in a GP surgery in Bradford on Avon. Due to the nature of the recruitment we are seeking someone who can start immediately if possible.

The Age UK Wiltshire Living Well Service has been proven to improve the wellbeing of older people and improve their general health. The project provides non-medical support to older people living with long-term health conditions. The role will involve having a conversation with the older person to establish what is important to them and from that conversation identify and agree a number of realistic and achievable targets that would enhance their life.

The successful candidate then works with the client and/or assigns a volunteer to accomplish the goals that have been agreed. As well as having the ability to engage with older people effectively this role requires people who are self-starters and able to work on their own initiative. The successful candidate will have energy and enthusiasm and an ability to monitor and develop the project. A positive approach to team work is essential as the post will lead a group of Age UK Volunteers and be part of the multi agency team in the GP Surgery.

Whilst this role is temporary for 6 months to cover staff absence there is the potential for other similar roles to become available within Age UK Wiltshire at the end of the term.

The nature of this post will require travel within a specified area therefore a requirement of this post will be access to and the use of a reliable car and a current driving licence.

This role will be subject to a Disclosure and Barring Service check.
