Support to reduce your risk of falling
Worried about falling? Feeling a little unsteady on your feet? Our Falls Prevention Service provides practical, physiotherapy-led support to help you regain balance and confidence.
This is a free service for people 65 and over who are living in the borough of Hillingdon or registered with a Hillingdon GP. It is designed for people who are independently mobile, with or without a walking aid.
What the service offers
Our specialist team will visit you at home to assess how you’re getting around and identify any risks that could increase your chance of falling. We will:
● Carry out a home visit to assess your mobility, health history & any previous falls
● Identify any balance or strength concerns using standard mobility tests
● Review your home environment and provide advice to reduce trip hazards
● Offer small pieces of equipment or mobility aids to help you stay independent
● Provide supervised exercises to improve strength and balance
● Accompany you outside as you regain your confidence walking outdoors
We aim to help you stay safe, active and independent in your own home for as long as possible.
Who is this for?
● People 65 or over
● Living in the borough of Hillingdon or registered with a Hillingdon GP
● Able to walk independently, with or without a walking aid
● Concerned about falling or feeling unsteady
Please note: This service is not suitable for people living in care homes or those with degenerative conditions such as Parkinson’s.

FAQs
Do I need a referral from my GP?
No referral is needed. You can contact us directly if you live in Hillingdon or are registered with a Hillingdon GP.
Can I still use this service if I already use a walking frame?
Yes. As long as you are independently mobile with or without an aid, you can access the service.
What kind of equipment do you provide?
We may provide small mobility aids and equipment to help you move around more safely. This depends on your assessment and needs.
Is the support available long term?
This is a short-term, preventative service. We aim to assess and support you over a number of visits, helping you to reduce risk and improve confidence. If you need longer-term help, we can advise or refer you on.
