Get help and support after leaving hospitalWe offer free short term help and support for older people leaving hospital, after an illness or in a crisis.

The aim of these services is to support people to remain at home and lead as active and independent a life as possible. We can offer free social and practical support for up to six weeks.

Such activities and support may include help with tasks such as shopping, light housework or dog walking. However the most important role is befriending, helping to alleviate the isolation that many older people experience. We also offer encouragement to help people to become involved in activities in their community.

There are two teams:

North-east Shropshire Interdisciplinary Team (NESIT)

The north-east Shropshire Interdisciplinary Team (NESIT) covers the Whitchurch and Market Drayton areas. The team consists of health and social care professionals, including physiotherapists, occupational and speech and language therapists, social workers and nurses.

Age UK Shropshire Telford & Wrekin is very much at the heart of the NESIT team; the coordinator and volunteers have a unique role in filling the gaps that other services cannot fill, which are equally important to gain the confidence and independence of the service user.

South Shropshire Home from Hospital team

The south Shropshire Home from Hospital team consists of two Age UK Shropshire Telford & Wrekin workers, supported by a team of volunteers.

The team can visit people on the hospital ward prior to discharge, and will assess the older person's needs before introducing a volunteer to support the older person in their own home.
