About The Role

Age UK West Sussex, Brighton & Hove (AUKWSBH) is looking for a compassionate Take Home & Settle Coordinator to join our dedicated Independent Living Team, based at East Surrey Hospital in Redhill.

This is a fixed-term role until 30 June 2026, with the possibility of becoming permanent if future funding is secured. You’ll play a key part in supporting older people as they leave hospital, ensuring they feel safe, comfortable, and cared for as they settle back home.

Working alongside our Senior Coordinator and Head of Independent Living Services, you’ll help shape and deliver a vital service that makes a real difference in people’s recovery and wellbeing.

What You’ll Be Doing

No two days will be the same! You’ll be:

  • Promoting the Take Home & Settle service across hospital wards.
  • Taking referrals from healthcare professionals and assessing suitability for discharge.
  • Supporting individuals on their journey home, offering reassurance and a friendly face.
  • Using your own vehicle to safely transport patients home from hospital.
  • Providing immediate practical help, from making a cuppa to checking food supplies and setting up essential equipment.
  • Carrying out simple home safety checks (with the client’s consent) to reduce risks.
  • Offering information and signposting to other helpful services.
  • Keeping accurate records and contributing ideas to improve the service.

Working Pattern

  • 18 hours per week on a 4-week rolling rota.
  • Includes one Saturday and Sunday every 4 weeks (with enhanced weekend pay).
  • Some flexibility required to meet client needs.

  • A driver with access to your own car – this is essential, as you’ll be taking clients home from hospital as part of the service.

Experience in health, care, or community services would be an advantage, but what matters most is your attitude, empathy, and willingness to make a difference.
