Community Gateway
The Community Gateway is available for everyone and provides access to a wide range of professional voluntary sector support and services. We have trained staff who can speak to you on the telephone or who can visit you at home if needed.
Our Approach
We believe in helping people stay independent for as long as possible. By offering early support, we aim to:
· Prevent problems starting or getting worse
· Boost confidence and wellbeing
· Help you stay in control of your own life
Local Gateway Outreach (GO) teams can offer individual and group support to help you to live safely and independently at home. We focus on practical, person-centred support that helps you feel confident, settled, and connected to your local community.
How we can help
We offer free flexible support and advice tailored to your needs, including:
· Connecting you with the right support We make sure your needs are understood and that you’re directed to the right support quickly and efficiently.
· Friendly Conversations and Guidance We take the time to listen and support you explore your options, connecting you to services and activities in your community.
· Support Settling Back at Home After time in a hospital or care setting, we can work with you to help you get back into your home routine comfortably and safely.
· Planning for Independent Living We work alongside you to build your confidence and support you to maintain or improve your independence.
· Getting Out and About in Your Community We support you to access local activities, groups, and services so you can stay connected and engaged.
· Help with Finances and Benefits We can guide you in accessing financial support and making the most of your income.
· Home Safety Checks Where needed, we can check your home to make sure it’s safe and suitable for your ongoing needs.
· End of life Support.
We can support people in the final stages of life, with dignity and compassion. We can offer emotional support for individuals and their families, helping to reduce isolation and ease some of the pressures associated with end of life.
· Support from Trained Volunteers
Alongside our professional staff, a dedicated network of volunteers can support people to rebuild confidence, stay connected to their communities, and feel less alone. Volunteers work hand-in-hand with Gateway staff, acting as trusted eyes and ears in the community. Volunteers are trained to provide a variety of support such as friendly face-to-face check-ins, telephone befriending, or accompanying someone on a walk to a local group.
The Gateway is open 8am to 8pm, seven days a week, 365 days a year – including Christmas day.Telephone lines are open 7 days a week 8am – 8pm.
