What is Community Gateway?

Community Gateway is a free service that supports you to live well and independently at home for as long as possible, by connecting you to the right help at the right time.

How has Community Gateway changed?

The Community Gateway has just got even better. As well as offering friendly advice over the phone, we can now visit even more people in their homes and help with a wider range of issues, making it easier to get the support you need.

Our friendly team offers one simple point of contact for a wide range of charities, health and community services that can provide assistance and practical help.

Who can use Community Gateway?

Community Gateway is for everyone to use. Whether you need help at home, or for a family member, neighbour, or the people you support, our friendly teams take the time to listen and explore your options.

How long does it take to get help?

Many people will have a plan in place within three days of contacting our teams.

We offer free, flexible support and advice tailored to your needs, including:

• Support settling back at home
After time in a hospital or care setting, we can work with you to help you get back into your home routine comfortably and safely.
• Planning for independent living
We work alongside you to build your confidence and help maintain or improve your independence.
• Getting out and about in your community
We support you to access local activities, groups, and services so you can stay connected and engaged.
• Assistance with finances and benefits
We can guide you in accessing financial support and making the most of your income.
• Home safety checks
Where needed, we can check your home to make sure it’s safe and suitable for your ongoing needs.
• End of life support
We provide compassionate support for people in the final stages of life, and for their family members and loved ones, offering a listening ear and practical help to reduce feelings of isolation during this difficult time.

Support from trained volunteers

Alongside our professional staff, a dedicated network of trained volunteers helps to rebuild your confidence, stay connected to the community, and feel less alone.

Volunteers are trained to provide a variety of roles such as friendly face-to-face check-ins, telephone support, or accompanying someone on a walk to a local group.

The Gateway is funded by NHS Cornwall and the Isles of Scilly Integrated Care Board (ICB) and Cornwall Council.
