Community Gateway Outreach Worker (Enhanced End of Life Support)

We are seeking an additional team member to join our Gateway Outreach team, who supports adults through person-centred, strengths-based conversations focused on “What Matters” to the individual.

Using the My CAW (Measuring My Concerns and Wellbeing) assessment, this role will involve co-producing with the client, Personalised Support Plans that promote self-management of health and wellbeing, early intervention, and prevention. The role involves short-term, timely support, empowering individuals to access appropriate statutory and voluntary sector services and regain control over their lives.

In addition, the role includes enhanced capability to sensitively support individuals who may be approaching the end of life. This support is non-clinical, addressing social, emotional, and practical needs, complementing clinical care to ensure dignity, compassion, and connectedness.

Reports to:       GO Locality Team Leader

Place of Work:  Based in Falmouth Community Hub, Killigrew Street, Falmouth, TR11 3QP   

Due to the nature of the work, travel is an essential part of the role therefore a driving license and access to a car is essential. You will need to be able to travel independently to and from client’s homes as and when required.

Salary:               £25,907 per annum (pending review)

Hours of Work: 37 hours a week, Monday to Friday, with the requirement to undertake, with notice, occasional weekend, evening and bank holiday support cover from time to time.

Duration:          12 months fixed term contract

DBS:                  This role is subject to an Enhanced DBS check

Since 2022, Age UK Cornwall & The Isles of Scilly, Partners (including Chaos Group, Volunteer Cornwall, Cornwall Neighbourhoods for Change and others) and Integrated Care Board have been working together to improve access to services, with the aim of reducing demand on acute services.
