Age UK Cornwall and the Isles of Scilly work in partnership to provide Hospital and Home Support Services throughout Cornwall. Supported by NHS staff, local community hubs and care providers, we focus on reducing admissions by providing practical solutions, person-cantered support, and preventative plans which improve physical, mental, and emotional wellbeing, alleviate anxiety, loneliness and social isolation.
Who we are
We are active in your community and able to offer assistance in Hospital and when you get Home. Our teams support people and their families. Our aim is to provide you with choice, control, and independence.
Last year, with support from a range of local partners, the Hospital and Home service helped over 1,200 discharge from Hospital safely, providing over 8,000 hours of support.
What we do
We are here to support you and your loved ones, our offer includes;
*Pre-discharge visits.
*Home environmental checks.
*Assistance with equipment collections.
*Personalised assessments
*4 to 6 weeks post-discharge support.
*Signposting to community activities.
*Falls prevention & wellness support.
*Support for caregivers and families.
*Help accessing funding and benefits.
*Companionship services.
*Warm meals (where applicable).
*Gardening and Home support solutions.
*Hospital aftercare services.
*Community transport services.
*Access to a network of Community Hubs.
Our Impact

At the heart of this initiative lies a robust support network and partnership, which includes; Kernow Community Gateway, Volunteer Cornwall, The CHAOS Group, NHS providers, and the Community Hub Network —all dedicated to providing personalised wrap-around care to individuals and families in need.
